The forms below are required items that will be needed when you come visit our office. Please feel free to complete these forms ahead of time.

We have made forms available in different formats (Adobe PDF & Microsoft Word). If you are experiencing problems please call us Toll Free: 877-984-9777 or email us and we will be happy to fax or mail you a copy.

You may either fill out the forms on your computer or print out the forms and fill them out by hand. To fill out forms on your computer, download the Microsoft Word Version. Press on the first gray text box and fill in the information. You may move to the next gray box by using the "Tab" Key. Save the file on your computer, and email the form back to us at info@texaslasik.com.
